Blended Case Management

                 What is BCM?                      

Blended Case Management (BCM) is a program that helps people with special mental health needs coordinate community-based psychiatric, medical, social, and educational services.

Blended Case Managers help those who have or are at risk for developing a serious mental illness or emotional disorder get the best treatment and support available in our area.
 
        Who is eligible for BCM?           
 
Any child, adolescent, or adult resident of Erie County that is in need of a high level of mental health service. Examples include but are not limited to: recent mental health hospitalization, involvement with child welfare services, placement in a residential treatment facility or significant difficulty managing life/social stressors.

We will help set up a recovery plan according to your personal goals and the intensity of your needs. We will link you to services in the community to help you
reach your goals and ensure you are receiving the care you need and deserve. The goal of the BCM program is to maximize your ability to live a full, healthy life.
 
     BCM is here to help YOU with:     
  • Obtaining mental health and medical treatment.
  • Finding suitable housing.
  • Securing benefits.
  • Accessing services in the community.
  • Problem resolution.
  • Linking to Transportation Services.

 

All eligible clients for the Blended Case Management program must have a referral form made out by a community provider.
